is it bad to run on a head gasket that needs to be replaced or just replace it right away?

scenario, lucky I caught it in time on a buddys car......
he was complaining of no heat, yet car was slightly overheating....would be ok if he put water in it once a month.......
Once car was shutoff, the residual pressure in the cooling system would force liquid back into cylinder number one. Liquid doesn't compress and when he went to start it it just grunted like it had a dead battery. New battery and it did the same thing, called me.
Pulled the plugs to crank it over, 6-8 foot fountain from #1, 2-3 foot from #2, hopefully no bent rods.
